Information on address change for our existing registered patients
For patients moving to a new address (Permanent or Temporary) outside of the practice boundary.
This a guidance for our existing patients who move outside our practice boundary and is applicable when the practice receives communication that gives their address as outside our practice boundary. If this information is not correct then the patient should immediately contact our reception team to correct this.
If you have indeed moved to a new address which is outside our practice boundary, it would be necessary for you to sign up with a GP practice closer to your new address. This is so that you continue to get full scope of GP services to meet your health needs including any requests for home visits.
When patients move outside a GP practice boundary, it is not possible to provide certain services such as home visiting service or certain urgent or complex care and this could affect the health care needs of patients.
This also applies to patients who temporarily move out of our practice boundary to stay with relatives or a care home/respite care facility outside our boundary for any period of time and who have complex care needs and/or where a visit may be required. Your health care needs will be best served by registering with a local GP as a temporary patient.
If you have any difficulty finding another GP please contact Primary Care Support England on 0333 014 2884 or via their website at https://pcse.england.nhs.uk/contact/. The NHS Choices website also provides information on GP services in your area and can be accessed at www.nhs.uk/service-search/.
Once you sign up with a new surgery, they will arrange for your medical records to be transferred.
If you feel that our information on your address change is incorrect and you are currently living at an address within our practice boundary, please contact the surgery to correct your details.
